The enumerations begins all with a rectangle independent from their level. Particularly if there is a picture or a table between the enumerations. I think it is better to use different signs for enumeration, depending on the level in the enumeration. (rectangle filled, rectangle unfilled, circle filled, circle unfilled). I know, this fact depends on at the OMG-Template (it concerns not only the BPMN-Document, I think).
