The spec doesn't say what should happen if I extract from an
Any with to_fixed() and the digits and scale don't match what's
indicated
by the TypeCode.

Proposal:

Add the following para as the second-last para of 1.16.4:

    For successful extraction of Fixed values with to_fixed, the
    digits
    and scale values supplied to the constructor must match the values
    in the Any's TypeCode; otherwise, the constructor fails and
    subsequent extraction with operator>>= returns false.
