Summary: The XSD fragment for difference elements in 4.5.6 uses XSD:IDREFS for 'target' which does not allow reference to external elements using href. The intention is clearly to allow external refercnes: section 4.5.6 starts "The Add class represents an addition to a target set of objects in this document or other documents." And 4.12.4 has an example (which is not valid acordign to the XSD fragment).
